- Complete and submit via mail the Request for Change of Address(Individual). - It is important to keep your records up to date as important information/documents are mailed to you. Please note: a new license will not be generated. You will have to request a duplicate license.
My company has or will have a new address
- Complete and submit via mail the Request for Change of Address(Principal or Branch Office). - There is a processing fee of $25. Be sure to list the employees that will be affected by the move. If additional space is needed, attach a typed list.
I will be working for two companies
- Complete and submit via mail the Notice of Dual Employment form. - You must notify the Board within 10 working days if you change employers.
I lost my pocket/wall license
- Complete and submit via mail the Affidavit of Lost License/Registration - There is a $2 processing fee per copy. Please allow 3 - 4 weeks for the delivery of your license.
